Home Health Aide and Homemaker

Certified Home Health Aides

Part time and per diem. Flexible day time and evening hours Monday through Friday, as well as weekend and holiday coverage. Responsibilities include providing personal care, light housekeeping and/or meal preparation and socialization for clients in the home. Requirements: Nurse Aide certification with Home Care experience. And excellent customer service skills a must.

Nursing

RN
Part time or full time, daytime hours, Monday through Friday, as well as weekend and holiday coverage. This position p
erforms appropriate evaluations of client progress and initiates prompt action when any change in the client’s condition is noted or reported. In addition, this position administers treatments and medications according to established policies, procedures, and practice. Requirements: Diploma in Nursing from a National League of Nursing accredited school; licensed in Connecticut and Massachusetts; minimum of one year of medical/surgical experience; and home health experience preferred.

LPN

Per diem, daytime hours, Monday through Friday, as well as weekend and holiday coverage. This position is responsible for evaluating client progress and initiating action when necessary. In addtion, this position administers treatments and medications, monitors progress toward goals, and coordinates services as necessary.  Requirements: Diploma in Nursing from a National League of Nursing accredited school; licensed in Connecticut and Massachusetts; minimum of one year of medical/surgical experience; and home health experience preferred.

Non-Nursing Clinical/Rehabilitation

Rehabilitation Supervisor

Full time, daytime hours, Monday through Friday. This position oversees and supervises daily management of patient rehabilitation care, including employee caseloads and scheduling. In addition,  this position assures compliance with agency standards and productivity goals and acts as liaison between management, therapists, and nursing staff. Requirements: Completion of an approved Physical Therapy program. Licensed in CT/MA. Four or more years of clinical experience in Physical Therapy required.




Speech/Language Pathologist

Flexible daytime and evening hours, Monday through Friday, as well as weekend and holiday coverage. This position is responsible for performing appropriate evaluations of client diagnostic speech, language and swallowing progress as well as administering speech therapy treatments. Requirements:  Licensed Speech Therapist in CT and MA. Three to five years of clinical experience in speech therapy. Experience in a Medicare home health agency preferred.

Registered Dietician

Flexible daytime and evening hours, Monday through Friday, as well as weekend and holiday coverage. This position develops written nutritional care plan based on the assessment of the client’s physical, psychological, and age-specific needs. Provides nutritional information and instruction to clients as necessary. Requirements:  Bachelor’s degree in dietetics or nutritional science. Registered Dietitian with The Commission on Registered Dieticians/The American Dietetic Association. Experience in home health care preferred.
