From I-84, take Exit 67. From I-84 East, turn left at exit ramp. From I-84 West, turn left. Follow Route 31 to stoplight. Turn right onto Route 30 and follow to intersection with Route 190. Turn left (west) and travel approximately 10 miles to the Hazard Trade Mart Plaza, located on right. Johnson Professional Associates Mental Health is located in the plaza, where Stella's Restaurant is also located.

From I-91, take Exit 47E to Route 190 East (Hazard Avenue). Drive past the intersections of Phoenix Avenue, Freshwater Boulevard, and Palomba Drive/Middle Road. Go past the Johnson Memorial Medical Park. After you pass the Medical Park, watch for the Hazard Trade Mart plaza on your left. Johnson Professional Associates Mental Health is located in the plaza, where Stella's Restaurant is also located.
