PATIENT INFORMATION
Pre-Operative Preparation
Thank you for planning your outpatient surgery at Johnson Surgery Center (JSC). The information below will help you to prepare for your visit:
- A registered nurse will call you a few days prior to surgery to tell you the time to arrive for your procedure, briefly review your medical history, verify your current medications and allergies and ensure that you have arranged a ride to and from JSC. If you have not spoken with a member of the clinical staff 24-hours prior to your surgery, please call the JSC Patient Information Line at (860) 763-7662.
- When you receive your pre-operative call from the nurse, please take note of the time you need to arrive at JSC and the approximate time you can plan to be discharged. Please also note instructions regarding eating and drinking prior to your surgery. Failure to follow these instructions could result in your procedure being cancelled.
- If you are taking diet pills, please speak with your surgeon regarding discontinuing the pills until after your procedure.
- Pre-operative testing paperwork should arrive at JSC 24 hours prior to surgery. If testing is not completed within Johnson Health Network, please provide the clinical fax line number to your medical doctor and/or laboratory and ask that this information be faxed to us.
- The JSC Business Office is available to answer any billing questions prior to surgery. If you have any questions regarding your specific plan coverage, benefits, co-pays or pre-certification requirements, please call your insurance carrier. On your day of surgery, we ask that you bring in your insurance cards and any applicable co-pays.

Patient Rights
- The patient has the right to appropriate, considerate and respectful care, regardless of the source of payment for his/her care.
- The patient, or an appropriate person in his/her behalf, has the right to obtain from his/her physician complete information concerning his/her diagnosis, treatment and prognosis in terms he/she can understand. He/she has the right to know by name the physician responsible for directing his/her care.
- The patient has the right to receive from his/her physician the information necessary to give informed consent prior to any procedure or treatment. The patient also has the right to know the name of the person performing the procedure or treatment.
- The patient has the right to refuse treatment to the extent permitted by law and to be informed of the medical consequences of his/her action.
- The patient has the right to privacy concerning his/her own medical care program.
- The patient has the right to expect that all communications and records pertaining to his/her care shall be kept confidential except as required by law or on the instructions of the patient.
- The patient has the right to expect that, within its ability, the surgery center will make reasonable response to his/her request for services and must provide evaluation, service and referral as indicated by urgency of the case. When medically permissable, a patient may be transferred to another facility only after he/she, or the appropriate responsible person, if he/she is unable, has received a complete explanation of the needs for, and alternatives to, such a transfer.
The patient has the right to be advised if a surgery center staff member or other group proposes to engage him/her in human experimentation, and he/she has the right to refuse to participate.
- The patient has the right to obtain information about any relationship of this Surgery Center to other healthcare and educational institutions insofar as his/her care is concerned.

- The patient has the right to expect reasonable continuing care and to be informed by his/her physician of his/her healthcare needs following discharge from the Surgery Center.
- The patient has the right to examine and receive an explanation of his/her bill regardless of source of payment.
- The patient has the right to know which Surgery Center rules and regulations apply to his/her conduct as a patient.
